Together we make a difference!

With the generosity of our community, PRC has touched thousands of lives in 2025, providing essential care, education, prevention, and support. From HIV and Hepatitis C services to food and housing assistance, health coverage, prevention programs, and more, our team works every day to remove barriers and create a healthier, more equitable community.

Hear directly from one of our clients about how your support makes a real impact.

2025 Impact Snapshot

PRC’s programs and services reach people wherever they are, whether it’s testing and prevention, essential resources, housing support, or mental health care. Below is a snapshot of the ways we’ve served our community in 2025.

Outreach, Education, and Testing

In 2025, we conducted 297 Outreach, Education, and Testing programs, providing thousands of free HIV, Hepatitis C, and STI tests, along with vital resources and basic necessities.

Street Outreach

Our donation-funded Street Outreach program meets primarily unhoused individuals downtown multiple times per week, offering food, water, socks, testing, and other essential resources, and following up wherever they are to connect them with care and support.

HIV Care & Viral Suppression

PRC now serves 451 clients living with HIV, with 95% maintaining an undetectable viral load, preventing progression to AIDS and transmission of HIV.
The national undetectable average is 65% (CDC).

Housing Assistance

More than 60 clients living with HIV receive support with rent and utilities, ensuring stable housing so they can focus on their health and personal growth.

Food & Essential Resources

Despite funding cutbacks, our Gregory L. Manifold Food Pantry remains a crucial resource for clients, ensuring hundreds of individuals and families have access to food, cleaning supplies, and personal care items.

Health Coverage

Through ongoing guidance from our case managers, 95% of clients are enrolled in health insurance, giving them access to medications, medical care, and supportive services, removing barriers so they can prioritize their health.

Hepatitis C Care

PRC’s dedicated Hepatitis C team identifies and treats new and existing cases, guiding clients through testing, treatment, and ongoing support. Roughly half of our clients have achieved a viral cure.*
Less than a third of individuals living with Hep C nationally are considered cured (CDC).

PrEP & Prevention

Our prevention team actively supports nearly 200 clients enrolled in PrEP, a medication that prevents HIV, empowering individuals to take control of their sexual health and stop new cases before they start.

Alliance Health Centers

Alliance Health Centers celebrated one year inside our agency, providing primary care to both PRC clients and the public. With providers just across the hall, clients can walk directly into care, removing barriers and expanding access to healthcare on Fort Wayne’s south side.

Additional Services

PRC also offers mental health counseling, support groups, translation services in Spanish and Burmese, protective barriers, free resources, and more. Our team is committed to meeting clients’ needs wherever they are, in every aspect of life.
